Pontiac Family Eye Care works with many insurance carriers and payment plans.

To see if we work with your insurance carrier or vision care plan, please call our office for assistance. Our friendly and informed staff will be happy to answer any questions you may have.

FOR OUR PATIENTS WITH EYEMED:
Since 2023: EyeMed will be paying YOU instead of us. Eye Exam Services & Frame/Lenses are paid up front at our office (or get a discount with our new membership plan).

We can help you submit for your out-of-network benefits & you will receive a check directly from EyeMed in 30 days.

*Note: Comprehensive exams for medical diagnoses will be billed to medical insurance rather than a vision plan.

OUR MEMBERSHIP PLANS:

Introducing our vision care membership plans. Think of them like a subscription for your vision care. They’re straightforward, give you more control over your care, and in most cases, cost less than self-pay vision insurance.

 

Note: We only accept Medicaid for children under 18 or for adults with medical eye conditions with residence in Livingston County. We have a waitlist for adults requesting routine eye care.
